“After getting my medical records, there are multiple Discrepancies and incorrect information in the notes, The Psychiatrist Jose’s own progress notes from our session explicitly state that Oxcarbazepine caused me “anger”. Despite having this documentation in front of him and me verbally confirming that the medication caused me problems because I had been taken off of it, he chose to keep me on it, claiming it would eventually help. This was a direct disregard for my reported adverse reactions and my physical safety, leading to unnecessary suffering and a complete breakdown of the provider-patient relationship. When I pointed out that the records contained blatant lies and requested an amendment, the office staff argued with me, spoke over me, and told me that “no such thing” as a record amendment exists. Because I stood my ground and advocated for my health, I was told to leave the building. This office is not only medically negligent by ignoring reported side effects, but the staff is also uneducated on patient rights regarding medical record corrections. I have terminated my care with Advanced Psychiatric Health, more specifically, with Jose Serrano because my clinical needs were consistently ignored in favor of heavy sedation that left me unable to function, drive, or care for my son with his homeschooling. Sedation is not treatment. The provider, Jose, repeatedly ignored my medical history and recommended medications I had already tried in the past and had negative side effects from. He documented that I "strongly declined" them ~ well, of course I declined them, because I have taken them before and they are not good for me. It is dangerous for a provider to ignore a patient’s medical history and then gaslight them by twisting their words to make it look like they are "refusing" medicine when they are actually refusing harmful treatments. He completely ignored my actual medical needs. I told him I needed a serotonin agent (antidepressant) to function and manage my OCD, PTSD, and panic attacks, but he dismissed my symptoms as "hypomania" just so he could keep me on sedatives and mood stabilizers. He was incredibly inconsistent, he would tell me at the beginning of an appointment that I could have a medication, only to take it away by the end of the same visit. He also previously put me on a medication only to take it away at the very next appointment. It made me feel like the rug could be pulled out from under me at any moment, and I was constantly worried if he wouldn't keep me on treatments that actually helped. This isn't just a misunderstanding, its a blatant disregard for a patient's trust and a confusing, unprofessional way to practice medicine. If you value a provider who is honest and stands by their word, look elsewhere. He even tried to claim I was asking for medication changes outside of appointments, when I was actually reporting severe symptoms and specifically asking for an appointment to discuss my treatment. He waited until I was in a full-blown crisis to finally admit he wasn’t the right fit. Save your time and find a provider who actually listens to your history instead of one who misdiagnoses you to cover their own clinical failures.”
