“began treatment in August while I was already in the middle of a Leave of Absence (LOA). Given my diagnoses and the seriousness of my condition, every time I needed to submit documentation to HR or the insurance company, it became an ongoing struggle. On three separate occasions, the provider submitted incomplete documentation—practically blank—sometimes including only a brief visit note. Required mental health and behavioral questionnaires, which were supposed to be completed at each visit, were never completed or submitted. Contacting customer service was consistently unproductive. Representatives appeared to be virtual assistants with limited authority, and the only response I ever received was that they would “message the provider” and follow up with me—something that never happened. Due to this repeated negligence and lack of accountability, my claim was denied three times, despite my diagnoses fully supporting medical necessity. Two weeks ago, I formally requested copies of everything that had been submitted on my behalf, along with my full medical record. Only then did I discover the extent of the errors and omissions in the documentation submitted on three separate occasions. Thankfully, I had other treating providers—including my primary care physician, orthopedic surgeon, and therapist—who were able to properly document and confirm my conditions with the insurance company. As a direct result of this provider’s lack of responsibility and empathy, I went three months without receiving any disability payments.”
